If it does not open, please save the file to your hard drive. Returning families do not need to fill out the student intake form for current students.

2. IMPORTANT! Save the fillable .pdf files to your computer. Do not attempt to complete the forms before saving. All information you complete before downloading the document will be lost.

3. Adobe Acrobat Reader must be installed on your device to complete the forms.

4. Fill out your information and save the completed forms to your computer. Please fill out one student intake form for every NEW student you are enrolling.
